Link to "company website."

I noted in your "webform application" that I do not have a website, yet your form asked for a URL anyway! As I said, I do not have a website! I don't need one. That's not what I am doing. I want to send low-volume, text-only service alert messages to known and consenting recipients. I do not market, advertise, or send unsolicited email. Why is this so hard for service providers to understand?! Websites are for people who have something to sell you - or steal from you. I'm doing neither. I just want to send text-only email messages to a handful of recipients on an as-needed basis.

Link to "LinkedIn profile."

As far as requiring a LinkedIn link -- LinkedIn is "profession Facebook." It is a garbage site full of scams, deceit, and nonesense. I stopped using LinkedIn in 2016. (You should, too.) If you want to verify that I'm a real person, ask for a copy of my DL like so, so many other service providers.

Question 7 - How do you get the list of your recipients?

Do not offer "K - Other" if it's not a viable choice. Answer "F" is the closest but still wrong; a more proper answer is "future client" or "test applicant."

Final Note

You have a typo in your closing page: "We'll reply you using email."
